Applications are invited from Registered and Enrolled Nurses who would like to take up the opportunity to join our dynamic Community Health team to work in District Nursing team in Mortlake and Terang. The primary role of the District nurse is to provide a person-centred approach focusing on maximising functional capacity, social participation, improved self-management strategies and coordination of service delivery to clients in their own homes.
You must be committed to providing evidenced-based practice, demonstrate an understanding of and commitment to nursing in the community setting and quality improvement. Applicants should have the appropriate skills including the ability to plan care, assess clients, and liaise with other care providers.
A current driver’s license is essential.
